HealthCheck accepts most major health insurance plans, and we bill insurance companies directly. Co-pay rates are equal to the amount set by your insurance company, regardless of treatment. Self-pay options are also available upon request. Please be sure to bring your primary insurance card at the time of treatment, and your secondary insurance card (if applicable). Be ready to provide the policy holder's name, date of birth, and social security number
